Cindy was a thirty-year-old loan officer who knew that she had a drinking problem. For instance, within the past six months she has felt the need to have a drink or two before going to work, four weeks ago she failed to pass a random urine alcohol test where she works, five months ago she got pulled over by the police for "driving under the influence", and last but not least, for around two months she has started to fail to remember what she says and does when she drinks with her buddies.
Similar to many other drinkers, Cindy's involvement with alcohol started out slowly and continued at this tempo for quite some time because every once in a while she engaged in casual social drinking. In fact, for almost two years, every time she drank, she made sure to drink responsibly. Something about her pattern of drinking, however, seemed to completely change when she got divorced from her husband.
Cindy got very depressed about the loss of her husband, and as a way to stop dwelling on her depressing emotions she decided to begin associating more often with some of her buddies who love to drink and have fun. It didn't take long, however, before her drinking increased significantly the more often she went to and drank at family get-togethers, sporting events, happy hours, dinner dates, and private parties with her friends.
In a word, she was partying just like everybody else in her group of friends without giving much thought to the alcohol short term effects and especially about the alcohol long term effects she has been exhibiting. Yet in the back of her mind she realized the facts about the negative alcohol effects on the body and on the brain and that she more likely than not required alcohol treatment but avoided the thought as much as possible.
One afternoon during her annual physical exam, her physician asked her if she drank alcohol. Not wanting to lie to her doctor, Cindy "came clean" and said that she frequently drinks more than she should. In fact, she stated that she routinely drinks in an abusive manner. Then Cindy informed her physician about her sadness. More to the point, she articulated that broken relationships many times initiated a dismal cycle of events typified by increased drinking which further resulted in more depressing feelings that, in turn, led to more drinking. And this is explicitly what happened when she and her husband got divorced five years ago.
Cindy's healthcare practitioner then told her the following: "I am not trying to make an impulsive judgment, but with your medical situation we may be dealing with two separate problems. As a result, I think we ought to make an appointment for you to get an alcohol dependency and alcohol abuse evaluation from my partner, Dr. Brill, who is an alcohol and drug abuse specialist."
"Whether your drinking circumstance is more related to alcohol addiction or alcohol abuse is not clear, but I think that further exploration is needed. Then I think we should make an appointment for you to get a psychological exam from another one of my partners, Dr. Manning, who is a counseling psychologist."
"I want to get a better handle on your sadness and see how much your depression and drinking are interrelated." Cindy showed her approval of her doctor's "game plan" and thanked him for his assistance and concern.
